The KE plus linear ESF system is an evolution of the original KE system. Although it is a versatile system that has functioned well over time, it has no advantage over the SF system. The KE plus clamp is designed to allow a wider range of positive threaded pins to be used than the original KE clamp, but clamp stability and security is not as good as the SF system. KE Plus is compatible with existing KE systems and is a useful addition to expand the range of the KE system. KE Plus clamps have a matt finish compared with the polished standard KE clamps. The KE Plus system is available in small, medium and large sizes, as indicated below. One important limitation of the KE plus system is that once the fixator has been applied to the patient, additional clamps cannot be added or removed from the connecting bar without removing the entire fixator, disassembling, and starting from the beginning. This is because the clamps cannot pass those already present on the connecting bar. This can be overcome by using the split clamp but this clamp is weaker and less secure than the standard KE plus clamp. Used to attach a connecting bar to another connecting bar. If possible, the fixation pins in the proximal fragment should be linked to the pins in the distal fragment via single clamps and a single connecting bar. This is not always possible because of the shape of the bones or the angle of placement of the pins. It may be necessary to connect the pins in the proximal fragment to a short connecting bar and the pins in the distal segment to another short connecting bar. Finally the two short connecting bars are joined by a long connecting bar using double clamps. The frame is not as rigid as the single connecting bar frame. Double clamps may be used to connect long bars proximally and distally, as in type III frames. Please note that two spanners are required when using Double clamps. Split Clamp allow the addition of an extra pin to the assembled KE plus frame without the need to dismantle the entire frame. This clamp is not as secure as the KE plus single clamp.
